How to establish your VA benefits

Welcome to Arizona State University!

Military service members will need to complete the steps below to establish their VA benefits upon being admitted to ASU.

You may complete your Certificate of Eligibility (COE) prior to admission. The process is simple. If you have questions about the status of your education benefits or if you need assistance completing your form(s) (VA Form 22-1990, 22-1995, 22-5490 or 22-5495), please contact the VA at 888-GIBILL1 (888-442-4551) to speak with an education case manager.

After registering for courses, you’ll complete two forms online and then send one to your academic advisor. Follow the steps below to get started.

Certificate of Eligibility (COE)

Email a copy of your COE to ptvcforms@asu.edu. Please visit ASU’s Veteran Benefits website for more information on getting your benefits started.

VA benefit responsibility

You only need to complete this once, prior to your first semester of enrollment at ASU.

VA enrollment activity form

You will need to complete one for every semester, after registering for classes or after making changes to your schedule to set up your benefits.

Prior credit evaluation

We will need this form by the end of the session. Make sure you have requested your military transcripts, in case you have any military credits. This needs to be signed off on by your military academic advisor. Once completed, email the form to ptvcforms@asu.edu.
